If you are making a Mailing Address Change and you are NOT the property owner, you MUST provide additional information before your request will be processed.  Please read the following:
 

· If the owner is deceased, provide a copy of the Death Certificate
 
· If you have Power of Attorney (POA), provide a copy of the document stating such
 
· If the property is in the name of a Business, Corporation, LLC, etc., provide a copy of the Articles of Incorporation showing your authority to request the change
